Poppy Seeds and False Positive Opiate Tests: Hair SPE with Clean Screen DAU

The Problem

Poppy seeds causing a positive drug test isn't an urban legend. It's a documented forensic toxicology problem. Codeine-predominant poppy seed products can push urinary codeine levels well above cutoff thresholds, creating false positives that have real consequences for workplace testing, military screening, and criminal justice.

The question isn't whether it happens. It's whether current thresholds are enough to prevent it.

The Study

Researchers conducted a 10-day dosing study with ten participants consuming poppy seed food products (lemon poppy seed muffins, everything bagels, cake, and kolach) previously identified as potentially codeine-predominant. Baseline urine and hair samples were collected, with urine sampled approximately every 24 hours across the consumption period. A final hair sample was collected within days of the last dose.

Codeine, morphine, hydrocodone, and hydromorphone were quantitated by LC-MS/MS.

The Extraction

Hair samples were powdered, then subjected to combined methanol and acidic extractions before purification by SPE.

Product used: Clean Screen® DAU (ZSDAU020), 200 mg / 10 mL

SPE Protocol (Hair)

  1. Condition with methanol, deionised water, and phosphate buffer (pH 6)
  2. Load combined hair extracts
  3. Rinse with deionised water, 0.1 M hydrochloric acid, then methanol
  4. Elute with methylene chloride:isopropanol:ammonium hydroxide (80:20:2)
  5. Evaporate and reconstitute for LC-MS/MS

Urine samples were hydrolysed and injected directly. Limits of quantitation: 20 ng/mL (urine), 40 pg/mg (hair).

Key Findings

Six participants (muffins and bagels) produced no positives. Four participants (cake and kolach) tested positive above 300 ng/mL at every post-dose time point.

At the 300 ng/mL cutoff: all positive samples contained codeine; 45% also contained morphine. At the DoD 2,000 ng/mL cutoff: 65% of those samples remained positive for codeine. No hydromorphone or hydrocodone was detected. Two hair samples tested positive for codeine at 41 and 51 pg/mg.

The critical finding: codeine reached 13,000 ng/mL in urine. Federal guidelines flag specimens for special review below 15,000 ng/mL when no 6-acetylmorphine is present, meaning these results sit dangerously close to the threshold where a false positive could go unquestioned.
